Registration
Joining iConquerMS™ in order to fill out surveys and provide personal and health information requires registration on this website.
Registration is open to individuals 21 years of age and older. If you are between the ages of 14 and 21, you will be allowed to join iConquerMS™ at a future date. The study will not be open to those under 14.
Participants must agree to the informed consent statement before registering. During registration, you will be asked to provide your name, email address, and password. We will also ask whether or not you have been diagnosed with multiple sclerosis (MS). Anyone age 21 years or older may register, regardless of his or her MS diagnosis.
At this time, all information is collected online. You may not register for iConquerMS™ through means other than this website.
Access to the registered parts of this website is controlled by a username (your email address) and the password you select. You may not register with an email address that is currently in use on this website. You are responsible for keeping your password confidential. Because of the potential for disclosure of personal information, you should not provide your User ID or password to any unauthorized third party. If you believe that there has been any unauthorized use of your account, you must contact the Accelerated Cure Project for Multiple Sclerosis immediately.
By registering on this website, you agree that the information you provide is accurate, complete, and up-to-date. You also agree to maintain and update this information as requested by iConquerMS™.
Only the Accelerated Cure Project for Multiple Sclerosis will have access to the name and email address provided in your registration. We reserve the right to access this information to reply to communications you send to iConquerMS™ or to enforce these terms of use.
We also reserve the right to disclose this information when special circumstances call for it, such as when disclosure is required by law or court order or when disclosure is, in our sole discretion, necessary to protect our legal rights, including intellectual property rights.
iConquerMS™ reserves the right to cancel your registration if we believe you are not acting in accordance with these terms and conditions.
You may withdraw from this study at any time and cancel your account. Access to this function is available by clicking on “Manage Account.” When you delete your account, your user name will be removed from any contributions you have made to forums. Withdrawing from the study and canceling your account means that you will no longer receive correspondence about this study and will no longer be asked to complete surveys. Any data you had submitted prior to withdrawal will be kept by the Accelerated Cure Project for Multiple Sclerosis, but will no longer be distributed to researchers. Any data that had previously been shared for research purposes cannot be retrieved from the researcher. Your contact information will be retained by the Accelerated Cure Project for Multiple Sclerosis.

You agree not to take any action that interferes with another user’s right to privacy or ability to use this website.
